What is RPM?

Remote patient monitoring is the collection and analysis of patient physiologic data that are used to develop and manage treatment plans related to chronic and/or acute health illnesses or conditions. While the Dignio Solution allows for capture of traditional RPM data we take it a step further to allow for collection of data related to validated health questionnaires, patient-provider communication, and the creation of multidisciplinary team care plans.

RPM codes are considered Evaluation and Management (E/M) services meaning only physicians or non-physician practitioners who are eligible to bill Medicare can submit for reimbursement.

RPM services can be provided by physicians, other qualified healthcare professionals (pharmacists, nurses, etc.) or clinical staff (medical assistants, technologist, therapists, etc.) under the general supervision of the physician.

CPT Codes

CPT 99453

Initial set-up and patient education on the use of equipment. Can only bill once per patient at the start of services.

CPT 99454

Capture of physiologic data by integrated devices that automatically upload and transmit data, bill each 30 days. Patient data must transmit 16 out of 30 days.

CPT 99457

Care management services by physician/clinical staff/qualified health care professional requiring interactive communication either in-person visit or virtual care visit with the patient in a calendar month, initial 20 minutes.

CPT 99458

Care management services by physician/clinical staff/qualified health care professional requiring interactive communication either in-person visit or virtual care visit with the patient in a calendar month, additional 20 minutes.

(Practices cannot bill more than 60 minutes of care management services per calendar month.)

CPT 99091

Outdated code so use with caution- replaced in 2020 with new CPT codes. Collection and interpretation of physiologic data (e.g., ECG, blood pressure, glucose monitoring), digitally stored and/or transmitted by the patient and/or caregiver to the physician or other qualified healthcare professional, qualified by education, training, licensure/ regulation (when applicable) requiring a minimum of 30 minutes of time, each 30 days.
